The Edmunds Middle School health office is open from 7:45am -2:45pm during every school day.  Our school nurse is available to administer medications, provide routine vision screenings, respond to emergencies, as well as the every day aches and pains of the students.  In addition, she can help set up doctor/dental appointments, work through insurance enrollments and generally assist in navigating the health care system.

Any student with a special health condition such as Asthma, Diabetes, Seizures, Cystic Fibrosis or Severe food or bee sting allergies needs to have a special health plan on file (see link on right side of this page).  Students requiring prescription medications during the school day need to have a medication permission form on file signed by both the parent/guardian and the physician indicating the medication, the dosage and any special instructions regarding administration. (see link on right side of this page.)  Any prescription medication kept in the health office needs to be in the original prescription container with student’s name, dosage and the provider’s name on it. 

A Burlington School District Secondary School Emergency Contact/health information form needs to be completed yearly for each student at the beginning of the school year. These forms provide the health office with important information about health insurance, emergency contacts as well as permission to administer Advil and Tylenol to students during the school day.
